New Eastern York Region North Durham Ontario Health Team announced

Through an Ontario Health Team, Triantafilopoulos said, patients will experience easier transitions from one provider to another, including, for example, between hospitals and home-care providers, with one patient record and one care plan.

In a meeting today at the Bracebridge Fire Department, MPP for Parry Sound-Muskoka Norm Miller represented Deputy Premier and Minister of Health Christine Elliot to announce the new health care program titled The Muskoka and Area Ontario Health Team (MAOHT). In advance of the rally, the Ontario and Ottawa Health Coalitions released a new briefing note about the potential impact of the cuts and restructuring on local access to care and services.

Albert Dupuis, the Co-Chair for Ottawa Health Coalition, says this initiative does not deal directly with Ontario's overcrowding in hospitals.

Eventually, home and community care functions will transfer to Ontario health teams, said Hayley Chazan, spokesperson for Elliott. They are falling through the cracks, waiting too long to get the care they need.

He noted, though, that the initial emphasis would be on improved patient and provider experience under current funded levels, rather than cutting health-system costs. This work will be implemented in phases and over time will provide care for everyone within the Brampton/Etobicoke and Area Ontario Heath Team's population.

The Halton team is scheduled to begin implementing its proposed programs and services in 2020 and will communicate with patients and families about the changes they can expect.

For now, 24 teams will be launched around the province as early adopters of the new system, including the Ottawa Health Team, which will include the region's major hospital, The Ottawa Hospital, as well as seven community health centres, Ottawa Public Health, Inner City Health, Bruyère, and others. Ontario Health Teams will play a critical role in connecting care providers and, in doing so, helping to end hallway health care.

"Together, we've reached a major milestone in the journey to connect care and modernize the health care system", Thompson said.

Ontarians can be confident that they can continue to contact their health care providers as they always have to access the health care they need.

"Today is a significant day for the future of health care right here in Halton and a significant day for the future of health care across Ontario".
