Because of Epstein's many ties to the rich and powerful - as well as his alleged underage sex trafficking involving his "Lolita Express" private jet and sex island - there's much conjecture someone was afraid the disgraced billionaire would sing so they had him offed.

He pleaded not guilty to the charges. But officials cautioned against taking a single finding from the autopsy out of context.

Of the two officers who would have had responsibility to monitor Epstein, one was not now a detention guard but was temporarily reassigned to that post, according to a person briefed on the matter.

It wasn't clear what the substitute's regular job was, but federal prisons facing shortages of fully trained guards have resorted to having other types of support staff fill in for correctional officers, including clerical workers and teachers. In documents unsealed last week, just a few hours before Epstein was found dead in his cell, Giuffre's lawyers accused Maxwell of "acting as a madam" for the financier, alleging that "multiple witnesses" had testified that Maxwell was responsible for "recruiting, maintaining, harbouring and trafficking girls for Epstein".

In the month before his death, Epstein was found unconscious with marks around his neck and subsequently placed on suicide watch.

But he was removed less than a week later and returned to a special housing unit where he was supposed to be checked every 30 minutes.

Mr. Epstein was alone in a cell when he was found hanging there.

In an email to Berman the same day, N'Diaye wrote that she could confirm that an internal investigation was conducted and completed in regards to the July 23 incident. The circumstances of his death are under investigation.

"I was appalled and frankly angry to learn of the [Metropolitan Correctional Center's] failure to adequately secure this prisoner", Attorney General William Barr told a law enforcement group Monday in New Orleans.
